Dear Mr Jugal
1.Delhi to Amritsar 8 Hrs BY CAR -01 day
2.Start Early at 5.30 AM from Amritsar to Mata Chintpurni via Batala-Srihargobindpur- Tanda Good Road -2.5 Hrs

3. Mata Chintpurni to Jawalaji -45 Min by car

From Jawalaji to Kangra-Mata Chamunda Devi temple - 3.5 Hrs- stay at Chamunda Ji or go to Dhramshala just 30 min drive

Next morning start early From Chamunda Ji to back Kangra and then to Naina Devi via barotiwala approx.-2.0 Hrs

From Naina Devi to Mansa Devi(Panchkula) straight without touching Chandigarh 2.0 hrs drive
From here it is your choice -if you do not want to see Chandigarh then go straight to Delhi

Well, Lets ee how the route goes:

Day 1
Delhi >> Amritsar (appx 470 km on NH1)>> Rest
Day 2
Amritsar >> Hosiarpur via Mehta, Shri Govindpur (105 Km on S HW)
Hoshiarpur>> Chintpurni Devi (45km on NH 70)
Chintpurni Devi>> Jwalaji (30 km on SHW) Rest at Jwalaji
Day 3
Jwalaji>> Nangal via Una Nadaun, Bangana, (abt 70 km)
Nangal >> Anandpur Sahib (23km on NH21A)>> Naina Devi (15 km around) Rest at Naina Devi

Day 4 Naina Devi to Chandigarh via Anandpur Sahib, Kiratpur, Rupnagar (abt 170 km on NH21) Chandigarh to Chandi Mandir, and Mansa Devi and return to Delhi late at midnight night.

Its going to be very very tight... coz mountain stretches will slow you down considerably where you may be able to cover 30 km. in 4 hours. Stretches near manikaran are demanding and will require sharp reflexes and certainly slow you down.

Your driver is also a human being and needs sufficient rest to rejenuvate his reflexes... my suggestion would be to add a couple days in the plan.

Not absolutely undoable but I think the 4 days is a bit cramped for so many places. Reminds me of a similar trip I made years ago.

By train
Delhi to Amritsar. (See Golden Temple)
Amritsar to Jalandhar >>

take cab/ taxi
to Jwalaji via Chintpurni Devi
return via Bangana>> Bhakra>> Naina Devi>>
proceed to Chandigarh >>
proceed to Chandi Mandir and Mansa Devi>>>
return to Delhi...

Now this can tell you will be pretty tight on your available time of 96 hours

Also read my Blog a lot can happen over a cup of coffee read HERE.
